bad tftp2 address

We have alcatel phones, which are giving an error "TFTP Bad address", i have checked their vlans, they are tagged in correct vlans, and they were working before, so the only thing changed was a firmware upgrade on the switch (HP), but we did  firmware upgrade on 10 more switches, and nothing happened to any of the voice phones on those floors, only on one floor are displaying that error.....
Any ideas?

Could spanning tree be pulling your leg?
show span

vlan-connectivity-check on vlan 123:
show mac vlan 123  (if connected some ports will show MAC-address, including uplink-ports)

anything in log?:
show log
if updating from a very old firmware, some miniGBIC's may show as counterfeit with new firmware

I 08/09/14 09:25:05 00435 ports: port E5 is Blocked by STP

on Show MAC VLAN it doesnt show E5, or E15 where phones are connected.

Do both ports act like this in log?
It seems E15 is down at the time, or disabled.

Could you please give output from
show running

What is IP of TFTP ?   -an IP helper could be missing

Took the port authentication 802.1x, deleted it from DHCP, disabled POE, re-enabled it.
Started working fine.
