I wear a lot of hats - Developer, Database Administrator, Help Desk, etc., so I know a lot of things but not a lot about one thing. Experts Exchange gives me answers from people who do know a lot about one thing, in a easy to use platform." -Todd S.

The issue is because the file uploader is enclosed within a IFRAME and there seems to be a bug with any file uploader that is enclosed in a IFRAME.
Workaround:  If you use the developer tools in chrome and open the file uploader in a new tab then it work just fine

Maybe update to Google Chrome or Silverlight  has broken the ability to add attachments when using the Outlook Web App (OWA). Which Version of Chrome you're using?
Google Chrome v 27 and above should resolves the attachment issue.

Please ensure your Google Chrome browser is up-to-date.

Exact this Version 37.0.2062.120 this issue is caused from removal of "showModalDialog" since Chrome 37.
TheFileUploader workaround was for an older version. Currently no fix available.
You could try the following:
Verify Silver Light installation by clicking the down arrow beside the ? mark [top right] & select About.
If you need to install Silver Light the 5th line down looks like this.
The required version of Silverlight is installed: No
Install Silver Light & Restart. Now the 5th line down in About looks like this
The required version of Silverlight is installed: YES

or just switch back to an older version

Uninstall Chrome, then you need an offline installer of an earlier version of chrome then follow these steps http://www.chromium.org/administrators/turning-off-auto-updates

Google scrubbing old Versions of chrome from the Internet

use chrome:plugins to see which silverlight is installed
getting latest here:  https://www.microsoft.com/getsilverlight/Get-Started/Install/Default.aspx

If that still not working you could only switch back to old chrome Version
or try this one:
http://www.afterdawn.com/software/network/browsers/chromeplus.cfm

use Firefox/IE instead for OWA Attachments until a fix is available
