Hi,
I'm using SQL2012.  I selected the device but no Database is shown in the dropdown and the OK button stays disabled.  On the top left it also shows error message or "No backupset selected to be restored).  How can I fix this?  Thanks.

Did you realize that you are trying to restore the version for SQL Server 2014 and not for 2012? Just check the filename.
