FILE UPLOAD WIDGET/ADD-IN FOR GOOGLE FORMS

I created a Google Form that posts to a Google Sheet.   Additional feature I need is to allow user to browse to and upload PDF's or images.    I would like to have the file uploaded and the filename added to a field in the Google Sheet along with other field data associated with the form.

At this point it is not possible to upload a file using Google Forms. Your best bet would be to try a third party form builder that would integrate with Google.

I had already tried formpl.us however the free version was littered with tons of ads for obvious reasons.   The project I'm doing is for our subdivision homeowner's association.  Looks like the free version of jotform.com will be more than sufficient for our needs.
