By default the "Maximum concurrent outbound connections" and "Maximum concurrent outbound connections per domain" are set by Microsoft to 1000 and 20.

What is the default value apply to? Is it for companies with a certain number of mailboxes? How would we best test what the optimal setting for that?

Default is 1000 and 20: http://www.serkanbark.com/2011/09/exchange-server-2010-hub-transport-role-properties-limits/

until you are not seeing any issue, leave the default value.
