How to attach a hand written memo to regular client record

I have an accounting package being used by an hospital. The receivables side of it
allows invoices to be prepared for clients.

I now have a need to be able to attach the hand written prescription written by the doctor
to my regular invoice record AND  to keep the 2 records stored together  possibly for later  retrieval.

Every invoice has a unique no and i would like  to have auto generated serial numbers for each prescription page

My application is written in Visual basic and can write to either  SQLServer or PostgreSQL databases

My question are:

1. What options do I have in selecting devices that can be attached to a laptop  and that allows hand written memo
to be save as part of a record ?

2. Is there any resource material  for reading out there that can help me achieve this requirement ?

Is there no current technology that allows the attachment of a device to a laptop and then  simply write on the device
and click "Save"  ??

If possible that would be my first choice unless of course its too expensive or difficult to implement.

After this consideration then I will think of having  to scan with its own attendant issues. The problem I have with scanning is that this involves another extra hand because the prescribing Doctor will not be the person scanning  what he already wrote

I was under the impression that the memo was already written.

Yes, there are many different writing tablets available on the market, with different features. Simply look for "writing tablet" on Amazon or Staples and make you pick.

I have never worked with that type of peripheral, but I suppose that a good one will be able to at least provide a standard image file or be controlled by an application.
