shortcode error in wordpress

I am trying to fix a site in wordpress that I did not create.  Not fun.  On this page below, in the right lower corner there is an error that says "contact form shortcode error: form contact us 1 does not exist".  How do I go about troubleshooting this?  Is it asking for a widget?

Oh dear god, why two of them? :/

(Not your fault, I know)

Okay, disable both and re-enable both and see if that fixes it.

If not, disable both, re-enable one and test. If no fix, disable both and re-enable the other.

Would it be possible that both are being used on the site?  Why would it call for the "short contact form"?  Maybe the long one is on the contact page?  Just guessing....

One of the forms had an update.  I got latest version to eliminate that being a problem.  Talked to client.  Instead of trying to fix lower right contact form we are going to replace it with a simple text link which goes to the contact page.  So, now I need to remove the code for this contact form.  will let you know if I run into any problems.

Sure, it's possible.  My point is that it shouldn't be.  

The shortcode failing is because whatever form is/was being called by that shortcode is either no longer there or misconfigured or something else of that nature.  Easiest thing to do is pick one of those two plugins and just build a new contact form in it and replace the shortcode with the correct one.
