AX 2012 Synchronization form disappears

I am trying to synchronize my AX 2012 database.  I start the process and it gets to the first screen with no errors and a few warnings.  I click continue and the process goes on for a while and then the form just disappears.  What could be causing this issue?

please can I ask what version you are running of AX012?? RTM, R2? and which CU you have installed? there was an issue in the earlier versions in the area.
