In simple terms, it's done by hosts communicating with each other (the Agent on each host), sending heartbeats via the network every 15 seconds.

Host isolation occurs, when it cannot communicate with any other hosts in the HA Cluster.
This is a long and complicated subject

I would start reading the King of DRS and HA - here

The differences between, 4.1, 5.0 and 5.5 are highlighted, also Master and Slave is discussed.

I would recommend any VMware Admin, purchase the HA Deep Dive books.
