Polycom blind transfers on FreePBX showing internal caller ID instead of external.

We are running Asterisk 10 and FreePBX 2.11. We have Polycom SoundPoint IP 450's and a VVX 400 phone. Whenever a call comes in from the outside and any of us transfer the call doing a blind transfer (both with the Polycom phone keys and using ## within the call), the transferred call shows as the internal extension caller ID name and number. When the user that the call was transferred to picks up the phone, they will then see the external caller ID information on their screen. I have tried changing the extension's "sendrpid" to "Send Remote-Party-ID header" and "Send-P-Asserted-Identity header" with the same results. Is there something I am missing?

Yep, this is the normal behavior..

Both of these options were set. I thought this was only normal behavior for attended transfers? I'm doing blind transfers.
I have always seen the issue with my installs going back to Asterisk 1.4 on both Blind and Live transfers.

It was working on this system until about a month or two ago and I have it working on another Asterisk 11 system for blind transfers. That's why I'm trying to figure out what broke and how to fix it
Just an update, this also works on all my other Polycom phones in the office. It seems to be that it doesn't work when I blind transfer calls to only two of my extensions. It works when blind transferring from those two extensions to other lines, or between any of the other extensions. I have made sure that "TrustRPID" is set to TRUE on all extensions and that all extensions have "Send P-Asserted Identity header" set. I don't see any configuration differences between the phones whatsoever.

Sorry, What I was asking is the firmware the same in the phones that work and the ones that don't ?   Not what the specific version is.
Oh, yes, same firmware all around. One of the phones that doesn't show the external caller ID is the same model as the rest as well: Polycom IP 450. The 2nd phone that isn't working properly is a VVX 400. All of the rest of the phones are Polycom IP 450's as well.
Guess, if you have the time some detail analysis with Wireshark is called for, otherwise maybe retire these two to the spares shelf and get two new ones to replace them..
I replaced these with two new phones and the problem is still happening. Any other ideas?
