I would like to create a pop up 'demo request' form using visual web optimizer (VWO).

Here are two articles that their support team linked to:

https://vwo.com/knowledge/vwo-custom-conversion-goal

But I don't know how to write the code to create a custom demo request form.

Also, I can't test it on the live site. Can I test it somewhere else like a wordpress.com site?

note: we are using concret5 CMS for our website.

Can someone please help out here?
