How to create forms in Outlook 2013

I have not found an effective way to create forms in Outllook 2013.  Any assistance is appreciated.

Are you talking about a form with VBA macros or a .oft Outlook Template?

For a Template:
1- Create a new mail message and type what you want to use as your template in this new message.
2- Once you have finished typing the message, click the File menu, Save As, type the name of the file, and for the Save as type, select Outlook Template (*.oft). If you do not have this option it is likely because Outlook templates are not installed.
3- Click Ok.
4- Close the e-mail, when asked if you want to save the e-mail, say No unless you want to keep a draft of this message.

For a VBA form, it's more complicated. It will depend on why you want to put on your form.

The problems with VBA forms are that if your contacts are not all on Microsoft Outlook, the form may not work properly.

Many users are on gmail, others on Hotmail or Outlook.com, and more...

With oft Template, you will have better results if this is what you are trying to do.

I have used outlook.oft.  It is workable but I had seen more robust forms.  This better than what I had before - nothing.
