Citrix XenApp CSG - No Server Configured on Specified Address

I have Citrix WebInterface 5.4 (WI) and Secure Gateway 3.3.1 (CSG) running on Windows 2008 R2 and NOT a member of my XenApp 6.5 farm. The CSG is working internally over SSL, but not externally over the Internet. When I try to launch a published app remotely, I get the following error after logging-in:
Unable to launch your application. Contact your help desk with the following information: there is no Citrix XenApp server configured on the specified address

Our firewall only allows ingress SSL/443 and the FQDN is NAT'ed 1:1 and resolvable locally on the CSG server.

In WI, Secure Access mode is set to 'Direct', will re-test in 'Gateway Direct' mode....

yes,
with "direct" the client try to access the server (possible with a private IP) direct; without contacting the CSG.
"Gateway direct" should work, if the gateway is able to contact the server without NAT.
You can try to catch the launch.ica file at the client. there you can see the connection-settings including the gateway settings.
