Update PHP Page to use MySQli extensions

Hi, Im trying to migrate my PHP MYSQL scripts from      mysql_xxx to mysqli extensions.

The script is quite basic in that it just enters data into a MySQL database from an input form.

Could someone please modify it to use mysqli_xxx  extensions.
