Database Detail Design Document

Looking for some good, detail database design document template or any reference doc should work too, it's an Oracle database, but doc should be used for any other db too.

Please do share/direct, if any one having any idea/experience.

just try the following google search and you may see heaps of templates available including those Oracle products.
