I sugest to Doxygen.

Just like Ivan post. But I want to do that manually.
Think you can use Doxygen with some configuration. That means that only part of the process will be "automatic".

I suggest to review Doxygen manuals to see how it can meet your requierement.

In http://sourceforge.net/ you will find other tools similar to Doxigen:

Docutils: Documentation Utilities, AsciiDoc - text based documentation, and so on.
