Need IT templates for Change Management and Maintenance forms

I'm looking for some templates that we can utilize on service desk, namely, change management forms and a IT scheduled maintenance form.  

If anyone has a good source for where I can find such forms in order to modify e.t.c. that would be great!

I should have been more clear.  Looking for an IT maintenance type schedule that I can send via email to end users for upcoming maintenance.  The Change management form I'm looking for is a standard type like you would use in accordance with an ITIL or COBIT approved service desk framework or similar.

No, that we can deal with internally. Looking for something to send out for end users notifying them of a maintenance procedure coming up.
