Make editable PDFs - fill-in/check boxes

I want to open MS Word documents in OSX Pages, export as PDF and convert fill-in spaces (e.g. __Check Here) to interactive editable fill-in spaces.

Visitors must be able to check (or fill-in) entries, be able to edit (change) entries, add name/email and finally click icon to email completed form to predetermined address.

iSkysoft PDF Editor Pro for MAC seems to be a likely candidate for the job. Not so much looking for a specific product recommendation as assurance that this suggestion (or other) will do the job.

No need to use a Windows application - there is a Mac version of Acrobat available as well.
I started with Experts Exchange in 2004 and it's been a mainstay of my professional computing life since. It helped me launch a career as a programmer / Oracle data analyst
