Windows 8.1 pro hyper-v issue

I have a Windows 8.1 pro machine and I am trying to turn on the hyper-v.
I checked the system requirement and it is good.
But every time I am trying to turn on, the system reboots and it says "working on adding features" then it says "couldn't add feature, undoing changes".
And when I check it back it is off.

Make sure the virtualization acceleration features are enabled in the BIOS.

How was the OS installed? Original OEM image or via flash drive?

I checked the bios and it says CPU-VT is supported.
The OS was installed from a flash drive

Insert the flash drive into the same USB port and try the role install again. You may need to select the option to "choose installation source" in the dialogue box and set the flash drive as your source.
I am not quite sure "try the role install again" mean.
if you can explain it in more details, this will be great

Hyper-V is a Role in Windows. So, plug in the USB drive and go through and Add Windows Feature choosing Hyper-V. :)

It seems there something is preventing it from completing the installation process.
once it reaches %93 it starts rolling back.
Is the machine up to date?

I tried again and I am still getting the same issue!
There should be some Event Log entries that could indicate the source of the problem?
Do know where I can find it?
