Php upload

My upload php script works fine with other files when uploaded. But I when try to upload this file of about 4MB I get the following error. "Fatal error: ERROR: UNABLE TO UPLOAD FILE in Upload.php on line 134".

I have gone further to set some parameters in the php.ini file as ff
max_execution_time = 1800
max_input_time = 1800    
memory_limit = 1024M    
upload_max_filesize = 100M
post_max_size = 100M

But the error still occurs. Below is a segment of the script where the error is occurring.

In particular, I am wondering if this input control (line 191) might be part of the issue.

<input type="hidden" name="MAX_FILE_SIZE" value="$max_file_size" />

Also, have you used phpinfo() to verify that the php.ini settings are in effect?  It may be necessary to restart PHP or the server to get them into play.

The  "MAX_FILE_SIZE"  input control line was the issue. I left it out and when I put it back in, it worked like charm. Thanks for everything.
