mount-database -identity dbname -force

obviously a permissions issue or something has it open
verify the database is not actually mounted; if not, use handle to see what has the mdb file open

Have you got AV software on the machine? If so, does it have the correct exclusions for Exchange on it?
If not, then that is probably the cause. This is classic AV interference.
The only other time I have seen this is when someone tried to backup the database with a file level backup using an Open File Agent.
