I wear a lot of hats...

"The solutions and answers provided on Experts Exchange have been extremely helpful to me over the last few years. I wear a lot of hats - Developer, Database Administrator, Help Desk, etc., so I know a lot of things but not a lot about one thing. Experts Exchange gives me answers from people who do know a lot about one thing, in a easy to use platform." -Todd S.

Basic branching logic.

Reports only who did answer.

For anything fancy choose a free external survey service like Survey Monkey. The SP Survey offering is very basic.

Jamie:  Does SP have as one of the answer choices a "free form" type of option for the answers?  Meaning that it is not a Yes/No or True False question.  Can you also add a "comments" section to each question that will show in the report?

Comments would need to be an extra question.

Thanks Jamie,

Comments can not be made for a particular question?  You need create a completely separate additional question that would I guess state something like "this question is for comments to the previous question".   If no comments then skip the question I imagine but this setup doesn't seem very practical way of making comments to a question.

The Survey lists in SharePoint are not fully featured. I have no hesitation recommending other solutions for this.
