Mediaplaver in PDF

When I open this pdf the mediaplayer appears as a white book with an arrow in the upper left.  Is there a way to have the mediaplayer appear the way it looks after you click on the arrow?

I see only the normal .pdf frame--no white book with arrow, unless you mean the one for "Convert .pdf file to Word or Excel" in the normal .pdf tool bar.  And nothing to do with media player.

Regrets.  Still do not see it and I do not know what the "contact info" is.  But I am looking at the .pdf in Reader, not Acrobat.
Perhaps draw a circle around it in another screenshot.

Regrets. My PC renders your .pdf in Reader (I do not have Acrobat) and the book with arrow does not appear.
Perhaps take a screenshot (not .pdf) and post, ideally with a circle around the areas of concern.

Or hopefully someone with Acrobat will look at this thread.

Now I know what "contact  info" is.  But I still do not see anything to do with Media Player.  And no book with arrow.
