How do I add a "date" field in a PHP form?

I have a form that currently uses a plain text box to enable the user to input the date (see attached screenshot and file). I would like the textbox to be formatted as a date field, i.e., either with date formatting or using a date picker. What do I need to do to accomplish this?

Ray, the date input type only seems to work in Chrome. It doesn't do anything in Safari and Firefox.

Yes, that is the nature of HTML and web browsers - they are prescriptive and permissive rather than descriptive and restrictive.  

You may want to learn about jQuery "datepickers" to try for a cross-browser solution.

jQuery is rather old -- spaghetti code from the early part of the millennium, but it can still be useful for things like this.
