Outlook Calendar Appointment is being deleted immediately

We have several Outlook 2010 / Exchange 2010 users where calendar appointments are going directly to their deleted items.  I thought it was because they were set to "Accept automatically" and "Delete from inbox automatically", but that's not the case.  there are also no rules in place that would affect email flow for them.  Is this a glitch?

When stuff like this happens try the following...
- Does this happen in OWA as well
- Create a new Outlook Profile
- Run Outlook in Safemode (outlook.exe /safe)
-  Double check rules
- Check transport rules on the Exchange server
