I don't have a good theory about what is causing your insert issue, but it's apparently not the image file itself. In the attached Visio diagram I added a background page and then inserted the your logo with no issues. <br />

If you still can't insert the image, can you copy and paste it into your diagram?<br />

Cannot insert image file into VISIO 2013
