Add Text to the left of a group radio buttons HTML5

How do I add  text to the left of a group of radio buttons?  See the optionGroup class in my attached files.
The text I would like to add is:
Select a rating
   5 -  excellent, 4 good,
  3 -  average, 2 - satisfactory
  1 -  poor

How do I align the group of radio buttons with the text boxes (see my attached files), using CSS?

I am struggling with how to position elements and would appreciate your help.  I am newbie.

"The solutions and answers provided on Experts Exchange have been extremely helpful to me over the last few years. I wear a lot of hats - Developer, Database Administrator, Help Desk, etc., so I know a lot of things but not a lot about one thing. Experts Exchange gives me answers from people who do know a lot about one thing, in a easy to use platform." -Todd S.

Do you have a mockup of what you want to achieve exactly?

Just need to understand your requirement as your request to have everything next to each other will make for quite a wide section.

I would like:

Select Rating    5(.)  4(.)  3(.) 2(.) 1(.)  - radio buttons
Email                 text box
Comments       text area
What would your recommendation be as far as layout, since the one that have would make a wide section?

@cookiejar,

Thanks for the points. Could you give an explanation for the "B" Grade. (Please read grading guidelines here http://support.experts-exchange.com/customer/portal/articles/481419)

Default grade is an "A" - if you don't award an "A" grade it is expected that you give an explanation as to why you did not.

Thanks
