Word Press Captcha quit working

We are using the plugin Really Simple Captcha in a Contact Form 7 contact page.

The page has not been changed since 9/9/2014.

Please look at http://rain1.com/new-site/customer-support/service-request/. Note the captch text box at the bottom is empty. Thus, no one can submit a form.

See attached files of the form & the captcha_form.jpg.

I tried deactivation the Really Simple Captcha plugin & re-activating it. Still the same. I thought about downloading the latest version (the current one is NOT the latest), but I'm afraid that might screw it up worse than it is.

Any suggestions?

What is your downloaded  Really Simple Captcha plugin version? Have you try to remove it and reinstall the plugin?

I tried deactivating it & reactivating it, no change.

Note it was working last week.

I had already found that link; I cannot read it, type too small; did browser zoom 3 times.

I tried to install new version. I get the attached.

This is unrelated, but we are trying to move the site to a new host. There are issues with that too, but the form containing the captcha works correctly on the new host.

What can I look for in the working version that is not there or different in older version?

I solved this myself.

The allocated disk space on the web host was VERY tight (I knew this but I did not connect it). The host added 30GB to the site. It now works.

The reason it failed is because every time Captcha is invoked, it generates a NEW image of the captcha code; the disk space limit had been reached.
