I have a PDF registration form for our youth sports chapter. Additionally, I have an Excel spreadsheet that contains all registration information for all participants in the program.
I want to use the data in the spreadsheet to automatically fill in (mail merge) the PDF form and generate 1 form for each registrant.
I have searched the web and adobe help docs but haven't had any luck so far.
I am using Excel 2011 and Acrobat Pro DC on my Mac running OSX 10.10.4.
I have attached a copy of the registration form. The spreadsheet contains sensitive data, so I can provide an example upon request.
