QuorumGroup/FileShareQuorum *FAILED* - Exchange 2013 DAG

The DAG was setup some time ago, and I just started getting this error


Server01      FileShareQuorum      *FAILED*      File share witness resource 'File Share Witness doesn't have a file share configured. File share witness arbitration can't be performed until the file share witness share has been properly configured.



Server01      QuorumGroup      *FAILED*      Quorum resource 'Cluster Group' is not online on server 'Server1'. Database availability group 'DAG01' might not be reachable or may have lost redundancy. Error:
File Share Witness : Offline
 is offline. Please verify that the Cluster service is running on the server.

In the Failover Cluster Manager, I see the File Share Witness in Failed state with the error

Can you run below command on you FSW server.

net share

Run cmd as admin and the run above command. If it ask you to start server service, say yes and then check it

The following solution has been provided to resolve your issue: Unable to bring the FSW of the DAG01 DAG online.. A summary of the solution is detailed below.

    •    Symptom - FSW cannot come online
    •    Cause - CNO password out of sync with AD
    •    Resolution - Gave currently logged on user full permissions to the CNO and did a repair and then created a new FSW
