How to get audio in speakers and headset at the same time

I have RealTek Audio HD running on Windows 8.1 on a all in one Samsung PC.  I have enabled two different audio streams in RealTek, but even with the speakers as default, when the headset is plugged  the speakers do not play.

I have enabled Stereo mix, I have set headset as default and tried to send the audio to the internal speakers and that does't work either.

1) R click on your speaker icon, select 'Recording Devices' (if you have the sounds dialog up, it's the 2nd tab 'Recording')
2) R click in the white space, be sure that BOTH "show disabled devices" AND "show disconnected devices" are checked.
3) **Now this step is going to take a little bit of thought for each computer being used: on my laptop (HP Elitebook) I get a device "Stereo Mix", ENABLE this device.
4) Click on Properties, go to the tab 'Listen'
5) Check the box "Listen to this device", also change the drop-down box below "Playback through this device" to be your second audio device
-- To verify that you've done this correctly, go back to the Sound dialog tab Playback, you should now see both of your audio devices showing the little green graphs moving.

Did you do the step for "Playback through this device" to be your second audio device?

After enabling Stereo mix ,You Also need to check the check box "Listen to this device"  and choose Speakers

"Listen to this device" Check box which can be found By clicking properties of stereo mix and choosing the Listen Tab.

Thanks jerseysam,  I have green equaliser bar showing on both speakers and headphones, but sound only comes from headphones.  if I unplug the headphone the speakers work fine.

"Listen to this device" is Checked and I have both green speaker and headset equalizer bar showing sound, but only sound from headphones.
Have you ensured you have the latest drivers for the sound card?

Yes, did a online driver search from within MS and I had the latest.

I have restarted and tried multiple times.  It seems so close.

Even with the headset plugged in and the Stereo Mix disabled, Speakers set to default then there is no sound on the internal speakers (nor the headset either).
Usually you can't do this, as the internal speakers are turned off by a small switch in the headphone plug as soon as something is plugged into the plug.

Your only option could be if you use external speakers and your PC has plugs for both, the external speakers and the headphone.
