Best practices for Media Queries for Standard/Popular Devices


I would like  Expert advice on the best practices for media queries to target desktop and popular devices -- both landscape and portrait.

Included in my question is -- is there a "Cascade" of a certain "required" order when creating these media queries.  

Also is a separate stylesheet the best way to go for these type of media querties?

Besides using media queries to adjust the pages for different device "widths" there are some elements I want to hide on certain devices -- such as cell phones.

So if you Experts can advise, I will be splitting the points among all contributors.

My view - take a look at Bootstrap and Foundation to see how they have handled this. Given the proliferation of these two frameworks and the extent to which they are in use in the mobile world, they should give you a decent bit of insight into what is the best practice.
