What are the best practices for developing network diagrams

I am looking for the best way (best practices) to build a good and accurate network diagram and what is the best information to include? I realize the obvious as to add flow elements such as the modem-->firewall/router-->switches-->servers-->work stations-->etcetera, BUT just want to check with you experts on the very best practices in building one of these diagrams.

1. 0 Level Diagram <<< a brief overview of the whole buidling etc
2. 1st Level -- Dig a bit deeper
3. 2nd Level - Concentrate on Equipment
4. 3rd Level - Topology of the networks IPs locations etc
5. Further. I use Visio
