Record Website fields to database

I've tried researching, but most people seem to want to display table data to a webpage.
I have a php website with an existing MySQL database.
My existing shopping cart requires the user to enter: Name, Address, phone, etc. which I think turns away people. I really only need the Email and a password.  So I want to discard my shopping cart and display on a page:

Enter your Email:   _________________________________
again (make sure they match) _____________________

Enter Password:  __________________________________
again (make sure they match) _____________________
[SUBMIT]  (RECORDS FIELDS TO TABLE)

You just need to remove the unnecessary field from the code (DB request) and from the template.

Are you using a custom script or opensource script like opencart?
