Duplicate distribution points entry

There is a duplicate entry of same distribution point in SCCM 2012 R2 console. How to remove the one.

If yours a typical SCCM installation and not a custom, it's simple.

Open the console, navigate to Administration, then Overview, then Site Configuration. Click Create Site System Server and click on the server where the DP role is installed. At the bottom, under Site System Roles, right click on Distribution Point role  and click Remove Role

in console administration--> site configuration - servers and site system roles, I could able to see only one entry but in console administration -- > distribution point, I could see 2 entry for the same site. Multiple times I tried removing the role and installing back, No luck.
Under monitoring - Distribution status both entries are shows with Green tick mark. says healthy. Something strange!!

I tried following also without any luck
Removed the DP from parent site
Deleted the site server
Deleted the SMS registry from the DP server
Re-installed DP,

Are you restarting the server after deleting everything? The distmgr.log file records content creation, compression, delta replication, and information updates. It should be clean after a deletion and you perform a cleanup. Once the DP role is removed, you can manually clean up the folders that were created during DP installation on the DP server. When you setup the DP are you adding the SCCM site server computer account to the Local Administrators group on the box where DP role is being installed? Have you removed the computer account from local administrators group.

Restarting - yes
Removing all left over folder - only SMSSIG$ was there in DP - rest of them are deleted
DP computer is member of admin in Parent server and Parent serve is local member of DP server

Are you adding the SCCM site server computer account to the Local Administrators group on the box where DP role is being installed?

Is this a fresh SSM installation or an upgrade from 2007?

Is this a US version or ???

Obviously SCCM links back to an SQL database for status information. Are there any duplicate entries in that database?

Fresh Installation of SCC2012 R2, Now upgraded to SP1
It is US English Version, I am from India

When I Execute below command on all databases , Pri and sec server, I could see only one entry  
Select * from DPinfo where servername like '%servername%'

I have added 3 more DPs to the same PRI server and no issues

There is little background in this issue

This server has added to DP to one of my secondary server by mistake. I have installed and cleaned the server  using below method on all databases

Select * from pkgservers where NALpath like '%server%'
Select * from pkgstatus where pkgserver like '%server%'  
Select * from contentdpmap where serverpath like '%server%'
Select * from DPinfo where servername like '%server%'

I uninstalled DP and executed above query on all databases and found nothing

Installed DP and executed above query on all databases and found only one entry and SCCM client package

Dump the contents of SQL so that you can see all of the entries.

According to your post of 9/7/15 "Installed DP and executed above query on all databases and found only one entry and SCCM client package" it appears that your problem is solved. Why are you attempting to delete this question?

Did you dump the contents of SQL so that you can see all of the entries?  If so, what did you find?

due to my limitation in SQL I have not tried to dumb the SQL contents. I am little worried about live database.  Can you provide some links which explains me dumping the database contents  without disturbing existing one

You don't have to disturb the database, just export its contents to a TXT file.
