Failover on DAG causes service down

Dear members,

On a 3 node DAG Exchange 2013 have strange response during failover. DAG has two servers with active-pasive databases and third server is a lagged database copy.
During failover event (doesn´t matter what non lagged copy server has database mounted), cluster removes vote node for operational server and cluster goes down due insufficient members (only lagged copy server can vote and failover is disabled on that).

Have anyone this faulty?

Goto failover snap in and open cluster logs check for errors and post it here.

There is no errors on cluster.

I have found errors about Microsoft Exchange DAG Management service and Microsoft Exchange Replication service terminated unexpectedly, this causes databases dismount and lost of service:

1. Apply latest CU.
2. If any AV installed, make sure of AV exclusions.

Dear Zacharia, all Exchange was fresh install using Exchange 2013 CU9, and there is no AV on servers because is on testing stage.

Edit.
Doing more research remove node vote was caused by dynamic Quorum. I have try disabling dynamic quorum and discover that both Exchange services was stopping. Until restarted server is not online, services can´t start again.
Restarting lagged server causes crash but there is no database failover on this case. Restarting on one active database server causes failover and crash. When restarted servers come back, all databases are active on non-restarted server (suppose failover works until services crashes and goes down databases).

Finally open a support case with Microsoft. Install kb2862064 did the trick.
