The best approach is to get a kit that includes the screen. You need a pentalobe screwdriver to take the iPhone apart. It will be included and is good for one use. The screws are included also. Any sealer will be included. So get a screen replacement kit and you should be good to go.

If you look on Amazon for screen replacement kits you'll see the typical set of tools .. small philips 000 (cross-head) and small p2 pentalobe screwdriver - a couple of plastic tools to pry and separate the screen and a tweezers (unless you have delicate fingers).

A quick look in will find the step-by-step guide to replacing the front glass and most iPhone 5s replacement kits on Amazon and elsewhere will include these tools and plastic bits

iFixit is going to be the best bet for this. They have in-depth guides, tricks, tools, and parts that you'll need to replace this yourself. I've helped a friend replace the screen on their iPhone 4 before using their guides/parts and it was super easy.
0.8 Pentelobe Screwdriver, 000 Phillips head screwdriver, flathead screwdriver, plastic pry tool, good set of tweezers, suction cup, and PATIENCE. Most of these screens will come with an adequate set of tools to do the job.

This youtube shows all the tools that come with a screen kit, and how it is replaced... it is not "super easy",  you have to be very careful with steady hands and good eyesight.
If you do not have the confidence to D.I.Y. then get it done by someone who has experience.
