Installing PHP 5 with IIS 7 error

I keep getting HTTP 404 error (see attachment) when attempting to access phpinfo.php file under localhost.  I've several articles on setup for php and configuration of IIS with windows 7 no help so far.  I have php-5.4.45-nts-Win32-VC9-x86 installed.

Appears to been the issue turned on extensions and .txt was appended.

I now am getting internal HTTP server error.

This error (HTTP 500 Internal Server Error) means that the website you are visiting had a server problem which prevented the webpage from displaying.

I think it is an access violation but not sure how to remedy it. Windows is probably picky about which exe files to allow to run as cgi; maybe event viewer will tell you more. Isn't there an error code like c0000002?
