disk error 01 AX = 4280 drive 9F when trying to install ESXi 6 ent
I am trying to install ESXi 6 ENT and it starts to load an then after about 5 minutes or so, this error comes up.. We have 5 servers of which one is already running this version of ESXi so not sure why I am getting this error.

is this ISO, burned to a DVD ?
We have seen this issue when the ISO is burned to a DVD in some servers, but if using a CD-R, it's okay.
So I would try burning your iso to a CD-R.

yes this same cd was used on another host a few weeks ago and it worked just fine..

No but these host have ESXi 5.1 on them with SD cards so wondering if this maybe causing the issue??
I tried updating the HP firmware and such but still the same issue...

this is OEM HP version of 6.0 ?
have you removed the CDROM/DVD-DRIVE from one of the other servers it worked on and tried it.
You could manually reset/format the SD cards.

yes it is not a drive issue because it is happening on 2 host which have ESXi 5.1 on them with SD cards. So it must be have something to do with ESXi 5.1 being on the SD cards which is causing the cdrom install not to work. It starts to load for about 5 minutes and the yellow bar goes across part way, but then it stops and the disk error listed above comes up...

re-format the SD cards, or remove them, and test with the working SD cards.
seems odd though it cannot read the media. (original installer source files)

well the sd cards are good because they have esxi 5.1 on them now and work just fine. But when trying to install esxi 6 onto these same boxes with the same sd cards, no go..

Also as far as it reading the media, it starts to because we get the yellow status bar going across the screen and you do see some files being listed on the left side, but then after about 5 minutes or so, it just sits there and then the error pops up..
I guess I could take out the sd card and format it and then try the install again if there are not other suggestions for this or I could call VMware and see what they say..

well when I put the sd card into my laptop it shows the partition as esxi and says format FAT. So I guess that is what I need to format it as or should I do ntfs and then let VMware change it when I do the install of esxi 6?
ok I got the sd formatter but it does not see the sd card because the sd card does not show up as a drive letter on my computer. It shows up is disk mgr but not as a drive letter..

With the sd card having VMware on it, windows does not see it as a drive letter.
Because right now I cant format it with the sd formatter because it does not see the drive.

I think you may need to look for a different SD card re-formatter.
Have a look for the HP USB Formatting tool, and see if that helps.

Well had no luck getting the sd card reformatted..
So what I did was create a bootable usb stick with esxi 6 and once I booted from it, I was able to get ESXi 6 installed....
no I now have esxi 6 on all of them.. That was the goal all along. I had one new esxi 6 host and then added it to my cluster with the other 4 esxi 5.1 host, after I upgrade the vctr to 6 also of cource.
Then I installed esxi 6 via the usb stick onto each of the esxi 5.1 host one at a time via new install.

So faulty CDROM or MEDIA!
Which is what we have seen, when this fault occurs.

I thought the same but what was really strange here is that 2 out of the 4 host I tried with the cdrom, all did the same thing and then I also tried 2 different cd's, one from VMware and the other from HP..
This issue really had me pulling my hair out...
