Event ID 18456 - SQL Server 2014

This error occurs when the IIS site is reported as down, and the database cluster does not automatically failover the database. Running a network capture, but a main architectural issue is that the witness server for SQL Server 2014 resides on the mirror server, and Microsoft strongly recommends it being moved to a separate server.  Has anyone experienced this issue before?

By Microsoft's article this error is due to a failed logon.
Are you trying to connect to the mirrored database instead the principal one?

The databases are coming up on both servers as mirrored.

Weird. That only happens when the servers or one server and the witness loses their quorum.

The more I research, the more it indicates an issue with the witness server, residing on either principal or the mirror server, which Microsoft strongly recommends be installed separately.
