are all insurance in usa come under [affordability act health care]

if an individual or company wants to get health insurance for themselves or employees, is it that all of them have to go through
the above mentioned? or is it optional?

for someone considering getting med insurance for first time, where do they start? are there lot of avenues or just one place/shop?
Any guidance will be helpful.

The ACA is a set of regulations for states, insurance companies, businesses, and individuals.
The implementation of these regulations varies enormously between states.

Here is information for Idaho (first state that came up).

Depending on the state and the details of your situation:
If you have or get a job, your employer may have health plans available.
If you must or choose to get insurance on your own, your state will have a regulated marketplace where you can purchase insurance.  Periods of open enrollment may be limited.

A number of states have state-level health care exchanges. We'd need to know which state in order to provide appropriate info.

For states that chose not to have their own exchanges, the Federal exchange is a good starting point:
