The breakpoint will not currently be hit. No symbols have been loaded for this document

I am using visual studio 2010 professional edition. In one of the web applications , that I am developing, I am not able to debug any page in the application because I am seeing the following message at the intended breakpoint "The breakpoint will not currently be hit. No symbols have been loaded for this document". I am using c#. scripting. Any thoughts to solve the problems

This can mean a couple of things:

You did not create a .pdb file when you built the project. (Are you in Debug mode? Debug does this by default, so if you didn't change anything, then you should be good.)
A referenced project is out of date as far as the build goes. In other words, the compiled DLL library that your project references does not match the current state of the source code. You would need to rebuild the library.

In the past 5 years that I developed several web applications, I did not encounter such a problem.
This is the first time, I am encountering such a problem. My questions are
How to create .pdb file.
How to  rebuild the libarary.
