ACCOUNT LOCK OUT

I am having account lockout issue with a user.

I have narrowed down the problem his Samsung galaxy s5 phone is causing the account lock out

we have removed exchange account , cleaned browser history but as soon as she turns on her phone her windows account gets locked up, cleared cached credentials and passwords from her laptop

she has not changed password on her mobile. what apps can be causing the problem, as soon as her phone is off her windows account never locks up
