The simplest way is to use a 3rd party to scan your mail, they specialize in scanning and removing spam, somewhere like mailguard..

Other than that I've had pretty good success with Spamassassin, although it's linux.. it's free and worked very well, but did take some tweaking:

Hi ukerandi,

Try use spamtitan. I had the same problem. In my system I am having exchange 2007 and I used all the free version of anti-spam methods but they did not work. However spamtitan helps me eliminating the spam emails from my enterprise system.

The Email supplier where my email finally lands uses Spam Assassin and it is very effective.

I use Mail.com as my primary mail input and their spam control is excellent.
Gmail as a host where you can collect mail is also very good.

Consider changing mail suppliers if you do not want to do it yourself.

Spam assassin too for me. It can use clamdscan for virus scanning though it is not its core role.  Also it is not just blacklisting based but rather has some learning capability to block spam more collectively (and not blindly). https://workaround.org/ispmail/jessie/spamassassin#comment-5325

The custom plugin support and scale it up for incorporating other intel source to up its visibility in  blacklist and whitelist filtering.  stop measures and integration with other Squid for the vote
https://wiki.apache.org/spamassassin/CustomPlugins

Specific to blocking it also does it for "backscatter" (via Virus Bounce Ruleset) which may be very relevant to the discussion. Those "scatter" is referring to mail you didn't ask to receive, generated by legitimate, non-spam-sending systems in response to spam

Spam (unsolicited commercial email (UCE) = junk email) usually sent as mass-mailed messages
and considered invasive by recipients. Spam causes inconveniences and has severe
impacts on productivity and thus is considered a business risk.
Antispam software/appliances are necessary and critical components of an enterprise’s security program, providing a mechanism to detect, I would recommend using detected services/appliances to detect spam such UTM Forti

You need anti Spam software. Outlook Junk mail is OK (barely). I also use an email supplier with good Anti Spam solutions.

More on leveraging user and already deployed appliance cum inherent email client features where possible. Some of these for consideration, though still good to get that anti-spam discussed..

Block spam at the gateway by checking domains in real time black hole lists
- black hole lists’ that contain IP addresses and domains from known spammers
- Lists of known spammer’s domains, and mail servers that are open to relaying hence susceptible to spammers sending mail via their mail server. There may be false positive in list so do quarantine first before straight deletion
e.g. Spamhaus Block List (SBL) (http://www.spamhaus.org/sbl/);
e.g.  Open Relay Database (ORDB) (http://www.ordb.org/)

Filter out spam based on email header characteristics
- Classify a mail as spam by checking email headers which is better than checking the actual email message content first.
- Check for anomalous trails e.g. Missing or malformed Message ID, Bcc: header exists (not in normal email), X-Mailer field contains name of popular spam ware, code and space sequence exists in email subject of the message (hide code from recipient with a large number of spaces placed before the code), text is entered outside the HTML tags so as to hide the code from the recipient, placing lots of HTML comment tags within the email body text..

Identify junk mail content
- Setting and selecting the filter on to detect messages that contain phrases such as CLICK HERE, FREE!!, there will be more of such, ...
- Need awareness training, to assess if mail is a hoax, share examples, conduct session and do not assume user are totally safe and savvy. Never send a reply to a spam mail.
- Not major scheme to block out spam but does helps to limit the negative impact of spam or hoax message having higher chance to pass your filters
