PowerShell -- upgrade from v4 to v5 ?

states that PowerShell v5 is doalbe on
Windows 2012 R2 instead of the built-in
v4 that displays when I do a "get-host"

What are the proper steps
to upgrade from v4 to v5 ?

I am using V5 on my Windows 10 machine, but I do not use it a lot, so V4 works for me as well.

Go here:   https://www.microsoft.com/en-us/download/details.aspx?id=48729 to download it. I read the article above and it is ambiguous in the sense that the download may be pre-release.

I suggest you consider waiting until it is presented in Windows Updates.

All you need to do is install Windows Management Framework Version 5 on the desired machine, it will upgrade the version of powershell

All you have to do is to ignore the temptation to try upgrading for now. PS4 works great, and besides on W10 there is no retail release yet, it's still a preview. You should have a very, very good reason to install.

Yes, as Qlemo indicates, WMF 5 is in preview right now.

Microsoft has indicated that it will only release PowerShell versions with the OS that it is hosted on. While Windows 10 is released and v5 of PowerShell exists on it, you cannot download or find an install for the full version of WMF 5 until Windows Server 2016 is released.  Then you will find a non-preview version to download and install. v4 is great and will do fine for you until 5 is released. If you want to play you can certainly download v5 preview.
