Windows Server 2012R2 HYPER-V MAC address conflict (event id 16945)

Hello!
After fresh install 2012R2, configure nicteaming (switch independent, Address Hash mode) and install hyper-v role, I see lots of warnings in windows log:
Event ID:      16945
MAC conflict: A port on the virtual switch has the same MAC as one of the underlying team members on Team Nic Microsoft Network Adapter Multiplexor Driver.
Some times users disconnects from VMs, and VMHost becomes unavailable.

get-netadapter tells me about dublicate MAC but i dont uderstand why it happend. Thanks for any help.

Thx for the right direction about Broadcom. HP Proliant have network card based on broadcom chip. I've done all you suggests and no more problems at the first look. I think the problem solved.
