Need a Kiosk - check-in program for a Mac Mini running Yosemite (but will upgrade to El Capitan soon. Maybe).

We would like to place that computer in our main lobby area and ideally it should let the user:
Their first and last name, phone number and email address, and how they were referred
(Craigslist, Indeed, friend, coworker, word of mouth, etc etc).

Free would be good. But if its highly recommended app, we will buy it.

Any recommendations?

xStand is a fantastic package for Mac OS. I've configured this for a couple of clients now and it customizes extremely well to the needs and growth-plans of small-medium businesses.

It isn't free, however the price is extremely reasonable for a package of this caliber. You are looking at $49.00 USD for the very basic package, and $149.00 for the effectively "unlimited" version (Up to 10 macs). It's a one-time fee - no annual renewals or version-upgrade costs.

Certainly worth consideration.
