Client's relative created an online form using bootstrap and wants to have me provide him with the code for the submit button. Not sure what that would be. Is it simply an email?

Yes, it is a small company that she has for hosting. The online form is complicated in that it bases the delivery charge on the location being delivered too. Again, I didn't do the form. It was over my head. Her nephew created it and now just wants "the code" to attach to the button. Which from what you and Dave say, isn't how it works.
I called tech support and they have an answering service. He didn't know if their hosting supported php or asp. I expressed surprise to say the least. He said, how should he know what they support, after all he is technical support! Hello? What???

After 8 minutes on hold was told that tech support could not answer my question. She said another level support will call me back in 24 hours after they have researched my question. Ugh!

Judging by that host's website, I have a feeling you are not going to get the answer you want.
I just looked at that Excel.net page. They are recommending IE6... That place is Seriously out of date and is not a 'modern hosting company. I didn't see anything that said they supported Any server side language. If they do not, then you can not finish that form there.
