Javascript using a combo box to pre-populate a field.

I am using Adobe Acrobat 10 and need to create a combo box (dropdown list) with the following items.
I am unfamiliar with how to write javascript.

for the combo box, the default will be a blank.
These items should appear next in this order:
Following Geri-Psych:
No Skilled Service:
Teaching and Training Successful:
Care Plan Management:
Observation and Assessment:
Teaching and Training Failed:
Therapy – Max Potential:
Skilled Nursing:
Therapy – Goals Met:

Following Geri-Psych:

Medicare covers medically necessary skilled nursing care needed on a daily basis.  You only need oral medications, assistance with your daily activities and general supportive services.  There is no evidence of medical complications or other medical reasons that required the skills of a professional nurse or therapist to safely and effectively carry out your plan of care.  Therefore, we believe that your care cannot be covered under Medicare. (B.1)

No Skilled Service:

Medicare covers medically necessary skilled care needed on a daily basis.  You only needed custodial care. This does not require the skills of a licensed nurse to perform the service or manage your care.   Since you needed neither skilled nursing nor skilled rehabilitation on a daily basis, we believe your stay is not covered under Medicare.  (B.2)

Teaching and Training Successful:

Medicare covers medically necessary skilled nursing or rehabilitation services you need including teaching and training activities for a reasonable time where progressive learning is demonstrated.  You needed only to be reminded to follow the physician’s instructions.  This does not require the skills of a professional nurse or therapist.  Therefore, we believe that this service is not covered under Medicare.  (B.3)

Care Plan Management:

Medicare covers medically necessary skilled care needed on a daily basis.  Because of your condition, you needed a skilled nurse from __________through __________to evaluate and manage your care plan.  Your condition has improved so the services you need can safely and effectively be given by nonskilled persons.  Since you no longer require skilled nursing or skilled rehabilitation services on a daily basis, we believe your stay is not covered under Medicare after __________. (B.4)

Observation and Assessment:

Medicare covers medically necessary skilled care needed on a daily basis.  You needed skilled nursing care beginning __________ to observe and evaluate your condition.  There is no indication of further likelihood of significant changes in your care plan or of acute changes or complications in your condition.  Since you no longer need skilled nursing or skilled rehabilitation services on a daily basis, we believe your stay after __________ is no longer covered under Medicare. (B.5)

Teaching and Training Failed:

Medicare covers medically necessary skilled nursing or rehabilitation services you needed including teaching and training activities for a reasonable time where progressive learning is demonstrated.  You received teaching and training for a reasonable time but demonstrated you were not able, at this time, to learn or progress to perform the activities ordered by your physician.  Therefore, we believe that skilled services are not covered under Medicare after __________.  (B.6)

Therapy – Max Potential:

Medicare covers medically necessary skilled rehabilitation services when needed on a daily basis.  While you required skilled therapy services from __________ to __________, the medical information shows that the skilled therapy services after that time is not reasonable in relation to the expected improvement in your condition.  In this case, since you do not need skilled nursing on a daily basis and the therapy services are not considered reasonable and necessary, we believe, your stay after __________ is not covered under Medicare.  (B.7)

Skilled Nursing:

Medicare covers medically necessary skilled care on a daily basis.  You only needed custodial care after __________.  Since you no longer required skilled nursing and did not need skilled rehabilitation on a daily basis, we believe your stay after __________ is not covered under Medicare.  (B.8)

Therapy – Goals Met:

Medicare covers medically necessary skilled rehabilitation services.  The medical information shows that the only therapy services you needed after __________ were repetitive exercises and help with walking and ADL’s.  These do not generally require skills of the supervision of a qualified therapist.  There was no evidence of medical complications which would have required that services be performed by a qualified therapist.  We believe therapy services are not covered under Medicare after __________.  (B.9)
