I have verified that the ODBC from the server can reach the SQL Server and the database successfully:

Microsoft SQL Server ODBC Driver Version 06.03.9600

Running connectivity tests...

Attempting connection
Connection established
Verifying option settings
Disconnecting from server

TESTS COMPLETED SUCCESSFULLY!

Any help would be greatly appreciated.

This is after installing KB3159706

I've done the post install steps but still not working, so I had to uninstall KB3159706 to make it working again.

Did you check if your server was patched with the pre-req before apply the KB3159706?

As per https://support.microsoft.com/en-us/kb/3159706

Pre-req: To apply this update in Windows Server 2012 R2, you must have April 2014 update rollup for Windows RT 8.1, Windows 8.1, and Windows Server 2012 R2 (2919355) installed.
